Job Title: LPN, Licensed Practical Nurse

Salary:$65,000 - $80,000 per year

This is a full-time position that requires a licensed practical nurse to work in conjunction with our interdisciplinary team to provide hands-on clinical nursing care to terminally ill patients.

Responsibilities:

• Administer nursing care to patients as needed, under the direction and supervision of the Registered Nurse
• Work with the Nurse Case Manager and Director of Nursing to develop and implement patient care plans
• Coordinate pain/symptom management programs with the participation of family and patient, attending physician and the Interdisciplinary Team
• Assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end of life issues
• Provide support and education for families based on the hospice philosophy of care
• Ensure all services provided are in accordance with the patient's Plan of Care
• Report changes in patient condition to the DON
