We are seeking an experienced Occupational Therapist to join our team at Providence Home and Community Care in Napa, California. As an Occupational Therapist, you will play a vital role in supporting patients in our Adult Day Health program, providing OT evaluations and developing and implementing individualized plans of care.
Key Responsibilities- Conduct OT evaluations and assessments to identify patient needs and develop personalized plans of care
- Develop and implement OT plans of care in accordance with PACE policies and procedures
- Collaborate with the interdisciplinary team to ensure comprehensive care and coordination
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
- Provide education and training to patients and caregivers on OT techniques and strategies
- Bachelor's Degree in Occupational Therapy
- California Occupational Therapist License
- Experience working in a geriatric setting, preferably in a PACE program
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
