Home Health Nurse Specialist

At Traditions Health, we are seeking a dedicated and compassionate Home Health Nurse Specialist to join our team.
About the RoleWe offer a competitive salary of $65,000 - $80,000 per year, depending on experience.
Key Responsibilities- Nursing Care Coordination: Provide skilled nursing care to clients in their place of residence, coordinating with the interdisciplinary team, patients, and their families.
- Care Planning: Develop, initiate, and revise individualized care plans to ensure quality and continuity of care.
- Documentation and Communication: Maintain accurate and timely clinical and progress notes, informing physicians and other personnel of changes in client condition and needs.
To be successful as a Home Health Nurse Specialist, you will need:
- A current State license as a Registered Nurse.
- One year of experience as a Registered Nurse in a clinical care setting, with home health experience preferred.
- Demonstrated ability to work independently and collaboratively within a team environment.
- Excellent communication and interpersonal skills.
