Job Summary

Mass General Brigham is seeking a highly skilled and experienced Registered Nurse to lead our Progressive Care Unit (PCU) team. As a Care Transition Specialist Lead, you will play a critical role in ensuring seamless transitions of care for our patients, working closely with interdisciplinary teams to coordinate referrals, placements, and follow-up care.

Key Responsibilities
  • Coordinate referrals and placements for patients transitioning from inpatient to outpatient settings, including extended care facilities, home care, and community agencies.
  • Act as a consultant to patients, families, and healthcare teams, providing guidance on available resources and services.
  • Develop and maintain relationships with community organizations, healthcare providers, and regulatory agencies to ensure optimal care coordination.
  • Monitor and evaluate the quality of care provided by extended care facilities and home care agencies, reporting findings to the Program Manager.
  • Participate in performance improvement initiatives, analyzing data to identify areas for improvement and developing strategies to enhance care coordination.
Requirements
  • Bachelor's Degree in Nursing or related field.
  • 3-5 years of experience in hospital discharge planning, long-term care, or utilization review.
  • Excellent interpersonal and communication skills, with ability to work effectively in an interdisciplinary team setting.
  • Knowledge of regulations, community organizations, and healthcare systems.
  • Basic typing and computer skills, with experience with personal computers and software desirable.
