Licensed Vocational Nurse

We are seeking a compassionate and skilled Licensed Vocational Nurse to join our team at Christian Care Centers- Fort Worth. As an LVN, you will play a vital role in providing high-quality patient care and contributing to the well-being of our residents.
Job Summary:The Licensed Vocational Nurse will be responsible for supervising direct care staff, performing direct care according to current standards of practice, state and federal regulations, and facility policy. This includes assessing and intervening in residents' physical and emotional conditions, communicating effectively with the care team, and carrying out physician orders in a timely manner.
Responsibilities:- Perform focused assessments and interim assessments for residents within scope of practice.
- Assess and intervene in residents' physical and emotional conditions, and report changes to the responsible party or physician as needed.
- Direct and assign work for direct caregivers to ensure quality care for residents.
- Communicate clearly and professionally with families, residents, professional providers, and teammates.
- Requisition approved materials, supplies, and equipment to carry out planned and ordered care.
- Maintain a safe environment for residents, adhering to infection control policies.
- Participate in ongoing training and in-services to improve knowledge and update skills.
- LVN degree from an accredited nursing program.
- Active LVN license to practice in the state of Texas.
- Minimum of one year in nursing experience in a long-term or other healthcare setting.
- 2+ years of experience preferred.
The LVN works in a healthcare community environment and must be able to move throughout the buildings and grounds as needed throughout the day. Ability to lift up to 40 pounds. Requires working under some stressful conditions to meet deadlines.

- Tasks may involve exposure to blood or body fluids.
- Tasks do not involve contact with blood or body fluids but could result in performing a Category 1 task.
- Tasks do not involve any risk of exposure to blood and body fluids.
Performs focused assessments, interim assessments for residents within scope of practice.
Assesses/Intervenes/Reports changes in residents' physical/emotional conditions, reactions to medications and treatments, attends and participates in care conferences, and reports to responsible party or physician as needed.
Assesses/monitors assigned residents' progress and gives feedback on individual interventions with care team.
Performs direct resident care requiring nursing skills (as approved by TBNE and within scope of practice); including, but not limited to IV, Wound Vacs, Injections, Gastrostomy Feedings, Ostomy Care, Tracheostomy Care, surgical drains.
Carries out/coordinates physician orders in a timely manner.
Participates and completes discharge-focused assessments and required patient/family teaching follow-up.
Directs/Assigns work for direct caregivers (CNA, CMA) to ensure quality care for residents.
Communicates clearly and professionally with Families, Residents, Professional Providers, and Teammates.
Requisitions approved materials, supplies, and equipment to carry out planned/ordered care; which may include medication refills being maintained in a timely manner, checking orders for errors and accuracy.
Maintains a safe environment for residents; including adherence to infection control policies.
Participates in ongoing training and in-services for improving knowledge and updating skills.
Performs other assigned tasks as requested and in accordance with policies and procedures.
Establish and encourage an atmosphere of optimism, warmth, and interest in residents' activity preferences and needs.
