Community Care Specialist

Helping You Heal LLC seeks a compassionate Community Care Specialist to join our team. In this role, you will work closely with adults with serious mental illness, providing treatment, rehabilitation, and support services.

Key Responsibilities:

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Conduct thorough assessments of patients' physical and mental health needs.
  • Provide education on medication management and psychiatric symptoms.
  • Coordinate with medical providers to ensure comprehensive care.
  • Maintain accurate records of patient progress and outcomes.

Requirements:

  • Registered Nurse (RN) licensure.
  • Minimum 1 year experience working with adults with serious mental illness.
  • Knowledge of psychiatric medications.

Estimated Salary: $60,000 - $80,000 per year.
