WORK ENVIRONMENT:
The Department of Elder Affairs delivers essential services through its Division of Statewide Community-Based Services, collaborating with local service providers to support a vital segment of the population. The primary focus is to offer home and community-based services to seniors at risk of long-term care placement due to frailty.

KEY RESPONSIBILITIES:
- Conduct thorough assessments of applicants for nursing facility placement and community-based services.
- Assess the Level of Care and suggest the most suitable placement options.
- Collaborate as part of an interdisciplinary team for Level of Care evaluations.
PRIMARY DUTIES:
This role involves significant responsibility in performing comprehensive assessments for nursing facility placements and community-based services. Key duties include:
- Coordinating and executing detailed assessments for applicants, evaluating their Level of Care, and recommending appropriate placements.
- Maintaining accurate medical records and ensuring timely data collection and reporting in compliance with established policies.
- Participating in interdisciplinary team discussions for Level of Care determinations.
- Ensuring completion of necessary processes before issuing Level of Care recommendations.
- Providing training to staff on relevant medical topics pertaining to the elderly and disabled populations.
- Engaging in continuous professional development to enhance knowledge and skills.
- Assisting in disaster recovery efforts as needed.
- Performing additional duties as assigned.
- Attendance is a critical aspect of this role.
REQUIRED QUALIFICATIONS:
- Must possess and maintain a valid and active Registered Nurse license or be eligible to practice nursing as per Florida regulations.
- Travel is a requirement; a valid driver’s license is necessary.

Preference will be given to current employees of the Department of Elder Affairs. A successful completion of a Level 2 Criminal History Background Check is mandatory for all selected candidates.
