Advanced Practice Provider - Nurse Practitioner or Physician Assistant

Job Summary:

We are seeking an experienced Advanced Practice Provider - Nurse Practitioner or Physician Assistant to join our multidisciplinary care team. As a key member of our team, you will deliver high-quality, patient-centered care to seniors and older adults with complex care needs in the comfort of their own homes.

Responsibilities:
  • Provide comprehensive, individualized care to patients with complex needs, including chronic condition management, urgent care, and transitions of care support.
  • Collaborate with our multidisciplinary team to deliver seamless care coordination, social determinants of health interventions, and behavioral health evaluation and management.
  • Deliver exceptional clinical care in-person and virtually, with a focus on building meaningful relationships with patients and their caregivers.
  • Participate in care team huddles, operational huddles, and organizational meetings to ensure seamless care delivery and continuous improvement.
  • Collaborate with external providers and clinical partners to coordinate care and sustain key partnerships with PCPs and other clinical partners.
Requirements:
  • Nurse Practitioner or Physician Assistant with graduation from an accredited health professions school and active national board certification.
  • Active licensure in the state of California. 103 NP licensure is a plus.
  • Active DEA licensure and state-specific controlled substance registration, as applicable.
  • A minimum of 3 years of experience in home-based care, with a focus on geriatrics and palliative medicine.
  • Certification in basic life support (BLS) is required.
  • Clinical experience caring for aging adults and adults with complex care needs is required.
