Travel Rehabilitation Specialist

We are seeking a skilled Travel Physical Therapist to join our team at Constellation Health Services.

The estimated annual salary for this position is $140,000. Our comprehensive benefits package includes 401(k), mileage reimbursement, health insurance, and paid time off (must meet minimum requirements).

As a Travel Physical Therapist, you will provide exceptional physical therapy services to patients by assessing and understanding their care plans. Your responsibilities will include:

• Evaluating patients' functional status and home environment for hazards or barriers
• Providing education to patients and caregivers on disease processes, medications, plan of care, and individualized treatment plans
• Maintaining and processing all documentation in compliance with agency standards utilizing the electronic medical record keeping system
• Submitting clinical documentation, time records, and other information in the required timeframe

To be successful in this role, you will need:

• A current and active PT license in the state of employment
• A graduate degree or greater in physical therapy
• A minimum of 1 year of experience in certified home health
• An in-depth understanding of end-of-life care principles and practices and certified home health
• Exceptional communication and interpersonal skills
• The ability to work independently and as part of a team
• A compassionate and empathetic approach to patient care
