Job Title: RN Care Coordinator

At Uphams Corner Health Center, we are seeking a skilled Registered Nurse to fill the role of RN Care Coordinator. This position will play a crucial part in our team's efforts to deliver exceptional patient care.

Estimated Salary: $65,000 - $85,000 per year (dependent on experience)

Responsibilities:
  • Conduct comprehensive assessments to identify patients' needs and develop personalized care plans
  • Collaborate with healthcare providers to ensure seamless transitions and coordinate ongoing care
  • Engage patients and their families in active care planning and provide education on managing chronic conditions
  • Facilitate referrals to primary care, behavioral health, and community-based social services as needed
Requirements:
  • Current, active MA Registered Nurse license
  • Experience working with Medicaid populations and Federally Qualified Health Centers
  • Strong communication and interpersonal skills, with ability to work effectively with diverse stakeholders
  • Ability to travel within assigned area to engage patients
