POSITION SUMMARY
As a Navigator, you will engage with referral sources from various organizations, including governmental and non-profit entities, to assist patients in accessing essential services at Tarzana Treatment Centers.
WORK SCHEDULE
8 Hours Per Day / 5 Days Per Week
RESPONSIBILITIES
This role involves conducting initial assessments to determine the appropriate level of care and services that support recovery efforts, as well as facilitating access to necessary health and social support services.
KEY DUTIES
1. Conduct initial navigation assessments via phone or in-person to identify individual needs and respond to referrals from various organizations, including county agencies and healthcare providers.
2. Deliver enhanced navigation services to connect eligible individuals with project services.
3. Follow up with clients to ensure all scheduled appointments and linkages are completed.
4. Maintain accurate and timely documentation of all interactions and navigation encounters.
5. Prepare and submit detailed reports regarding program activities, client demographics, caseload, retention rates, and other relevant information.
6. Demonstrate proficiency in the program's reporting database and generate necessary reports as assigned.
7. Exhibit professionalism and independence when engaging with clients in various settings.
8. Attend community meetings and represent Tarzana Treatment Centers appropriately.
9. Participate in outreach initiatives at community locations and in-reach at service sites.
10. Maintain a comprehensive database of clients served and outcomes for reporting purposes.
11. Attend agency and community meetings as directed.
12. Undertake additional tasks and responsibilities as assigned by the supervisor.
13. Assume leadership responsibilities when designated by the supervisor.
MINIMUM QUALIFICATIONS
Passion for working with individuals and families, demonstrating a non-judgmental attitude and positive regard for those facing barriers to health services. Experience in providing field-based services is essential. Strong interpersonal skills and the ability to work with diverse populations are required. Organizational skills, attention to detail, and creative problem-solving abilities are necessary. Proficiency in computer applications, including Microsoft Outlook, Word, and Excel, is expected. A valid driver's license is required.
EDUCATION/EXPERIENCE
A minimum of one year of experience as a Patient Navigator, or at least two years of relevant experience in fields such as Case Management or Benefit Specialty, or a Registration/Certification as a Substance Use Disorder Counselor, or a Bachelor's degree in Substance Use Disorder Studies, Psychology, or a related field.
