Pharmacist - PRN

We are seeking a skilled Pharmacist - PRN to join our team in Blanco, TX. As a key member of our pharmacy team, you will play a vital role in providing exceptional patient care and ensuring the safe and effective use of medications.

• Evaluate physician medication orders for appropriateness, potential interactions, and route of administration.
• Make recommendations for therapy changes to physicians and facility staff as needed.
• Maintain patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medications.
• Ensure laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
• Collaborate with the healthcare team to provide clinical and technical support as needed.
• Document all communication with physicians, nurses, and other healthcare team members in the medical record.
• Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
• Obtain and maintain required documentation in accordance with federal and state regulatory agencies and policy.
• Inform the Pharmacy Manager of medication-related errors.
• Assist facility/agency staff with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.

• Bachelor's Degree in Pharmacy; Pharm D preferred.
• One year experience in long-term care.
• Licenses/Certifications: As required by government regulations and business needs.
