About the Role

CareBridge Health is seeking a highly skilled Occupational Therapy Specialist to join our team in Massachusetts. As a key member of our Long Term Services and Supports (LTSS) team, you will play a critical role in supporting individuals aging in place and receiving home and community-based services.

Key Responsibilities:

  • Conduct comprehensive assessments to identify participants' needs and provide recommendations for supportive services.
  • Collaborate with Managed Care Organizations to ensure seamless delivery of services.
  • Develop and maintain strong relationships with program participants, MCO representatives, and interdisciplinary team members.
  • Stay up-to-date with home and community-based service systems, MCO processes, and state regulations.
  • Meet or exceed productivity standards and metrics benchmarks.

Requirements:

  • Unrestricted OTA, PTA, OT, or PT license.
  • 2+ years of experience in social service or healthcare related field.
  • Fluency in Spanish.
  • Knowledge of home and community-based service systems.
  • Ability to work in a fast-paced environment and adapt to changing workload.
