Responsibilities

  • Engage in classroom educational sessions covering critical thinking, evidence-based best practices, care planning, and care evaluation
  • Observe experienced RNs to acquire field knowledge and participate in group debriefing sessions
  • Collaborate in the community with a preceptor as part of a multidisciplinary care team
  • Provide individualized nursing and care management to patients in their residences or care facilities
  • Continuously assess changing patient needs and adjust care plans accordingly
  • Foster ongoing professional development through participation in in-services, training, continuing education, and relevant journal readings
Required Qualifications
  • Recent RN licensure
  • Nursing degree from an accredited program required
  • No previous nursing experience necessary
  • Ability to navigate stairs to access patient homes
