Quality Home Health is seeking a skilled and compassionate LPN Staff Visiting Nurse to join our team. As a key member of our home health care team, you will provide high-quality patient care and support to our patients in the comfort of their own homes.
Responsibilities:- Deliver efficient and effective patient care, ensuring the highest level of quality and satisfaction.
- Coordinate with our RN Team Leader to ensure timely completion of assigned visits and submission of required documentation.
- Establish and maintain effective visit routes, providing direct patient care in compliance with the plan of care.
- Communicate frequently with our RN Team Leader, physicians, and other team members to coordinate changes in patient care and ensure accurate documentation.
- Assist with the maintenance of a complete and accurate calendar book, including disaster codes, emergency information, and due dates for lab work.
- Complete supporting documentation in a timely and accurate manner, including visit reports, care plans, and progress notes.
- Be available for on-call and routine patient visits, rotating on-call assignments as needed.
- Participate in team conferences and provide input on patient care and progress.
- Oversee the timely requisition of patient supplies and verify physician licensure.
- Maintain confidentiality of business practices, activities, and personnel information.
- Attend inservices, seminars, and meetings as assigned by your supervisor or management.
- Licensed Practical Nurse, currently licensed in the state of Tennessee.
- Current valid driver's license.
- Ability to provide direct patient care and take on-call assignments.
- Working knowledge of the home health care program and previous experience providing home health visits preferred.
- Computer data entry and word processing skills desirable.
