Job Summary:

The Speech-Language Pathologist (SLP) will provide speech therapy services to patients in a home health setting. This role requires a Master's degree in Speech-Language Pathology and state licensure as a Speech-Language Pathologist in Texas.

Responsibilities:

  • Create, review, and update individualized patient care plans addressing specific speech and language needs.
  • Deliver speech therapy services and activities for patients with speech and language disorders.
  • Document patient records and clinical notes in a timely manner.
  • Collaborate with local teams and participate in staff development opportunities.
  • Communicate effectively with agency, clients, and Area Support Manager to ensure well-coordinated patient care.

Requirements:

  • Master's degree in Speech-Language Pathology from an accredited program.

  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA).
