Job Summary:

HexaQuEST Health, Inc. is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare organization.

Job Responsibilities:

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
• Collaborate with healthcare teams to develop and implement individualized patient care plans.
• Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and implementing interventions as needed.
• Maintain accurate and detailed records of patient interactions, treatments, and outcomes.

Requirements:

• Current LPN licensure in the state of employment.
• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
• Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities simultaneously.
• Basic life support (BLS) certification, with the ability to obtain advanced certifications as needed.

Work Schedule:

• Every other weekend required, with flexibility to work double shifts as needed.
• Must be available to work a variety of shifts, including days, evenings, and overnights.

Agency Requirements:

• Must bring a state or federal issued identification to the facility and wear a photo badge at all times.
• Agency staff must complete their primary series of COVID vaccination in order to be considered for any assignment.
• State specific mandated boosters are required and must be completed when eligible.
• Once the assignment has started, agency staff must follow the normal testing protocol of each particular center.
