Join Our Team as a Home Health LPN

Aveanna Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in the Sunderland, MA area. As a Home Health LPN, you will provide high-quality, patient-centered care to adult and pediatric patients in their homes.

Key Responsibilities:
  • Implement and coordinate care plans in accordance with physician orders
  • Conduct patient assessments and document care provided
  • Administer medications, treatments, and therapies as prescribed
  • Provide education and training to patients and family members
  • Collaborate with the healthcare team to ensure seamless care delivery
Requirements:
  • Valid LPN license in the State of Massachusetts
  • 6 months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience preferred
  • Ability to speak, write, read, and understand English
  • Ability to travel and reposition patients without assistance
