Job Overview

Fresenius Medical Care is seeking a highly motivated and compassionate Dialysis Social Worker to join our team. As a member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to provide psychosocial support and improve quality of life for individuals with end-stage renal disease.

**Key Responsibilities:**

  • Conduct comprehensive patient assessments to identify psychosocial strengths and areas of need.
  • Develop and implement effective care plans to address barriers to treatment adherence and improve quality of life.
  • Provide individualized counseling services to patients and their families, addressing concerns related to dialysis, transplantation, and end-of-life care.
  • Collaborate with the healthcare team to facilitate referrals to community resources, vocational rehabilitation, and other supportive services.
  • Maintain accurate and timely documentation of patient interactions and interventions.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.


**Requirements:**

  • Masters in Social Work (MSW) degree from an accredited program.
  • Licensure as a clinical social worker in the state where employed.
  • 0-2 years of experience in a healthcare setting, preferably in dialysis or nephrology.
