Hospice Social Worker

Job Summary:
We are seeking a skilled Social Worker to join our team at Karen Ann Quinlan Hospice. As a Social Worker, you will play a vital role in providing psychosocial assessments, developing care plans, and advocating for patients and their families.
Key Responsibilities:
- Perform comprehensive psychosocial assessments to identify patient needs and develop individualized care plans.
- Develop and implement care plans that meet the unique needs of patients and their families.
- Advocate for patients and their families, ensuring they receive necessary services and support.
- Provide emotional support and counseling to patients and their families, promoting emotional expression and well-being.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Document patient interactions, progress, and outcomes in a timely and accurate manner.
Requirements:
- Master's Degree in Social Work from a Council on Social Work Education-accredited program.
- Current licensure as a Social Worker in New Jersey.
- One year of post-Master's Social Work or counseling experience.
- Strong assessment and documentation skills, with ability to work independently and as part of a team.
- Excellent communication and interpersonal skills, with ability to provide emotional support and counseling.
- Proficiency in operating a computer, including Microsoft applications and electronic medical records.
