Join Our Team as a Skilled Nursing Specialist

We are seeking a highly skilled and compassionate Advanced Practice Provider to join our team at AlignMed Partners. As a Geriatric Nurse Practitioner, you will play a vital role in providing primary and urgent care services to our nursing center residents.

About the Role

The Advanced Practice Provider will work collaboratively with our Center Leadership Team to achieve our mission of delivering high-quality healthcare and everyday compassion. You will be responsible for providing medical care to our residents, working closely with our Medical Director and Attending Physician to ensure the best possible outcomes.

Responsibilities
  • Provide primary and urgent care services to nursing center residents
  • Collaborate with Center Leadership to achieve AlignMed Partners' mission
  • Work with Medical Director and Attending Physician to ensure high-quality patient care
  • Participate in ongoing education and training to stay up-to-date on best practices
Requirements

To be successful in this role, you will need:

  • A Master's Degree in Nursing with a concentration in Older Adult and/or Family Nurse Practitioner
  • A minimum of two years' experience working with adult or geriatric populations
  • An unrestricted professional license and DEA in states where practice allows
