We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team at Kids Care Home Health. As a Pediatric Home Health Nurse, you will play a vital role in providing high-quality patient care to infants, children, and adolescents in their homes.
Key Responsibilities- Deliver and supervise patient care through assessment, diagnosis, planning, implementation, and evaluation.
- Provide direct patient care according to the active plan of care, as given by the physician and other appropriate personnel.
- Administer prescribed medications, treatments, and therapies as ordered by the physician.
- Document timely, clear, and accurate clinical notes in accordance with the plan of care.
- Notify the RN Case Manager of any changes in the patient's condition.
- Provide and document education to patients and their families on specific details of diagnosis, treatment, and prevention.
- Collaborate with other professional disciplines to ensure that patients are receiving quality care.
- Current State or compact Licensed Vocational Nursing (LVN) License.
- One (1) Year Home Health Experience Preferred.
- One (1) Year Pediatric Experience Preferred.
- Current CPR Certification.
The position regularly requires bending, reaching, standing, stooping, sitting, twisting, talking, and hearing. The nurse must be able to push and pull 50 pounds, support 50 pounds of weight, and lift 50 pounds. The nurse must also be able to see information up to 24 inches away.
