RN, Home Health Nurse lll/Telehealth

Job Summary:
We are seeking a highly skilled and compassionate RN to join our Advanced Illness Management Transitions team as a Home Health Nurse III/Telehealth RN. This is a hybrid position that requires 50% remote tele support and 50% direct patient care in Central Valley, Roseville, and Modesto areas.
Key Responsibilities:
- Assess, coordinate, deliver, and evaluate in-home nursing care, including direct patient care, patient/family education, and transitions of care.
- Develop, implement, manage, and coordinate an optimal interdisciplinary plan of care that incorporates psychological, sociocultural, spiritual, economic, and lifestyle factors.
- Foster and maintain collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications.
- Adhere to policies, industry standards, best practices, and applicable laws/regulations and codes to promote a quality, highly reliable patient experience.
Requirements:
- Graduate of an accredited school of nursing.
- RN-Registered Nurse of California.
- BLS-Basic Life Support Healthcare Provider.
- California Drivers License.
- Valid Auto Insurance.
- 5 years' experience working as an RN in a home health or ambulatory setting.
Preferred Skills and Knowledge:
- Demonstrated proficient knowledge and competence with regard to nursing theories, concepts, and practices; medical terminology; anatomy; and physiology.
- Working knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations.
- Proficient knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record.
- Time management and organizational skills, including the ability to prioritize patient care, assignments, and work within standardized policies, procedures, and practices to achieve objectives and meet time-sensitive activities.
