Job Summary
We are seeking a Nurse Care Coordinator to join our team at DaVita Inc. As a Nurse Care Coordinator, you will play a critical role in coordinating patient care and ensuring that our patients receive the best possible treatment. This is an exciting opportunity to work in a dynamic and fast-paced environment where you can make a real difference in the lives of our patients.

About the Role
The Nurse Care Coordinator will be responsible for coordinating patient care from admission to discharge, working closely with our interdisciplinary team to ensure that each patient receives comprehensive and coordinated care. You will also be responsible for communicating with patients and their families, addressing concerns and providing education on treatment options and self-care techniques. Additionally, you will participate in quality improvement initiatives to enhance patient outcomes and satisfaction.

Requirements
To be successful in this role, you will need:
* A degree in Nursing (Associate Degree or Bachelor's Degree) from an accredited institution
* Current RN licensure in good standing
* Experience in a dialysis setting or other related healthcare field
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
