Job Title: Hospice RN

Traditions Health is seeking a compassionate and skilled Hospice RN to join our team. As a Hospice RN, you will provide high-quality patient care and support to individuals and their families in their homes.

Key Responsibilities:
  • Conduct initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of patients.
  • Develop, implement, and revise individualized care plans to ensure quality and continuity of care.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
  • Collaborate with the interdisciplinary team to coordinate care and services.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
Requirements:
  • Current State license as a Registered Nurse.
  • One year of experience as a Registered Nurse in a clinical care setting.
  • Home health experience preferred.
