VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our team in providing high-quality home health care to patients in Jersey City and Bayonne.

Responsibilities
  • Assess and evaluate patient needs, developing personalized care plans to promote optimal health outcomes.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and families.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Document patient progress, updates, and outcomes in a timely and accurate manner.
  • Identify teaching needs and develop strategies to educate patients and families on disease management and self-care.
Requirements
  • RN with a Bachelor's degree in Nursing, preferred.
  • 1+ year of supervised clinical experience as a Registered Nurse.
  • Excellent verbal and written communication skills.
  • Strong computer skills and proficiency in electronic medical records.
  • Excellent organizational and interpersonal skills.
  • Valid driver's license and reliable transportation.
