Position Overview:


At WellBe Senior Medical, we are dedicated to providing exceptional geriatric care through a model that emphasizes a Physician Led, Advanced Practice Provider driven approach. Our focus is on serving frail, poly-chronic elderly patients who are often underserved and face significant barriers to accessing healthcare.

We believe in bringing care directly to our patients, ensuring they receive the support they need in the comfort of their own homes.

Our comprehensive care spans the entire continuum, including chronic and urgent care in-home, hospital care, skilled nursing facilities, assisted living, palliative care, and end-of-life support.


As part of our care teams, you will collaborate closely with the patient's primary care physician to deliver high-quality geriatric medical care and social support, ensuring seamless coordination across all levels of care.

Key Responsibilities:
The Physician Assistant will embody the values and mission of WellBe in every aspect of patient care delivery.

Your role will involve adhering to state and federal regulations, as well as maintaining compliance with relevant certified agencies.

As an advanced practice provider, you will be integral to the care of patients in their homes, focusing on delivering compassionate support to those with both chronic and acute health conditions.

This position is essential to our interdisciplinary team, which is dedicated to providing comprehensive care that allows patients to remain in their own homes.

Key duties include:

  • Managing a full patient panel and delivering ongoing primary, urgent, and acute care.
  • Maintaining accurate and timely patient records.
  • Conducting regular follow-ups and ensuring patients receive necessary appointments for their health needs.
  • Working collaboratively with the broader interdisciplinary team to ensure high-quality, timely care for all patients.

Qualifications:


To be considered for this role, candidates must possess:

  • A Master's Degree in Physician Assistant Studies from an accredited program.
  • A minimum of 3-5 years of experience as a Physician Assistant, with preference given to those with prior healthcare experience.
  • An active, unencumbered Physician Assistant license in the state of New York.
  • Current board certification with the NCCPA.
  • BLS certification and an active NPI number.
  • Experience in home care, primary care, hospice, palliative care, or geriatrics is highly desirable.


Skills and Competencies:
Successful candidates will demonstrate:

  • Proficiency in conducting in-home visits and utilizing telehealth platforms.
  • Strong communication skills, both verbal and written.
  • Ability to collaborate effectively with an interdisciplinary team.
  • Commitment to delivering high-quality patient care and maintaining professional standards.

This role may require travel to various patient locations and the ability to lift and assist patients as needed.
