Key Responsibilities:

  • Deliver evidence-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients
  • Order and administer immunizations and injections as needed, including COVID-19, influenza, TB, and Depo
  • Conduct health screenings, behavioral health assessments, and educate patients on healthy lifestyle choices
  • Collaborate with other healthcare professionals to develop comprehensive care plans and ensure continuity of care for patients
  • Communicate effectively with patients, families, and caregivers to ensure positive outcomes

Requirements:

  • Master's degree level Family Nurse Practitioner/Physician Assistant program with current National Board Certification
  • State of Employment license to practice in the Advanced Practice Nurse role
  • Minimum of two years of medically relevant experience or equivalent
  • Basic Life Support (BLS) certification and ability to perform CPR and use an Automated External Defibrillator (AED)
