  4. Analyze patient outcomes and adjust treatment plans accordingly to achieve optimal therapeutic results.
  5. Foster collaborative relationships with healthcare professionals, home health nurses, and patients to optimize medication therapy and resolve related issues.
Requirements:
  • A Bachelor's of Pharmacy or PharmD degree from an accredited institution.
  • Previous experience in home infusion pharmacy.
  • A valid state pharmacy license.

Compensation Package

The estimated annual salary for this position is approximately $125,000, based on experience and qualifications.
