About the Position

We're seeking a skilled Nurse Practitioner to join our team, providing primary, urgent, and acute medical care to patients in their homes. As a key member of our care community, you'll work collaboratively with our physicians, social workers, and other healthcare professionals to deliver high-quality care and support to our patients.

Responsibilities
  • Provide primary care to patients in their homes, including initial screenings, physical examinations, and ongoing care
  • Collaborate with our care team to develop and implement comprehensive care plans
  • Order and interpret diagnostic tests, prescribe medications, and manage treatments as needed
  • Deliver palliative care and end-of-life care to patients and their families
  • Participate in regularly scheduled team meetings and contribute to the development of our care community
Requirements
  • Current licensure as a Nurse Practitioner in the state of [insert state]
