About the Role:
We are seeking a skilled Case Manager/QIDP to join our team at Swansea Skills Training Center. As a key member of our staff, you will be responsible for facilitating person-centered services that empower clients to achieve their goals and maximize their potential.
Salary and Benefits:
The successful candidate can expect a competitive salary of $22.10 per hour, plus a comprehensive benefits package that includes healthcare, paid time off, life insurance, short-term and long-term disability, and training and development opportunities.
Key Responsibilities:
- Client Relations: Promote a culture of person-centered services that advance the Company's mission. Ensure high client satisfaction by communicating effectively and addressing client needs.
- Person-Centered Programming: Develop and implement personalized plans to support client progress and well-being. Evaluate and document progress, ensuring compliance with regulatory standards.
- Employment, Training, and Integration: Supervise provision of facility-based and community-based services, including training and integration opportunities. Assist clients in achieving employment goals and objectives.
- Continuum of Care: Supplement staff efforts to ensure all client needs are met. Transport clients as needed for community outings, employment, or other appointments.
- Effective Leadership: Mentor and develop team members through training, problem resolution, and performance evaluation. Communicate regularly with staff on client updates and workplace changes.
Requirements:
- Bachelor's degree in human service-related field or Registered Nurse. One year of experience working with people with developmental disabilities.
- Certifications and licenses: CPR, 1st Aid, Crisis Prevention Institute (CPI), QIDP, and Direct Support Person (DSP) training provided by the company.
- Computer skills: Proficient with Microsoft Word, Excel, and Outlook.
- Background check and valid driver's license required.
Physical Demands:
- Frequently standing, walking, bending, squatting, reaching, and twisting.
- Occasionally sitting, kneeling, climbing stairs, pushing/pulling a rolling pallet jack, and lifting up to 50 lbs.
Estimated Salary Range: Based on location and industry standards, the estimated annual salary for this role is approximately $46,000 - $52,000.
