Job Summary

We are seeking a skilled Social Worker to join our team at Virtua Health. As a Social Worker, you will play a critical role in assessing, diagnosing, and intervening with patients and their families to address their bio-psychosocial needs and behaviors.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized plans of care.
  • Collaborate with multidisciplinary teams to coordinate patient care and ensure seamless transitions between healthcare settings.
  • Provide clinical social work consultation to physicians, staff, and other team members to optimize patient care and operations.
  • Refer patients and families to appropriate resources for abuse/neglect/domestic violence, alcohol and substance use disorder, and other biopsychosocial needs.
  • Perform crisis intervention, grief counseling, and bereavement support to enhance patient and family coping skills.
Requirements
  • Master's Degree in Social Work (MSW) from an accredited school of social work.
  • Current valid New Jersey State Social Worker License (LSW).
  • Excellent verbal and written communication skills, problem-solving, conflict management, organization, and planning skills.
  • Ability to understand medical terminology, diagnostic criteria, and communicate effectively with diverse populations.
