We are seeking an experienced Hospice RN to facilitate hospice and palliative care referrals from all sources and obtain reviews & coordinates the necessary information to complete the clinical assessment of the patient for the correct program & correct LOC within that program.
Key Responsibilities- Acts as a clinical resource for all members of the hospice/palliative team.
- Develops and maintains a working knowledge of all services available within the Home and Transitions Service Line.
- Completes comprehensive and accurate documentation timely and consistently that clearly reflects interventions performed.
Our Outpatient Hospice and Palliative Care team is dedicated to providing exceptional patient care and support. We believe in delivering compassionate and personalized care to our patients and their families.
