We are seeking a skilled Social Worker to join our team at SSM Health Saint Louis University Hospital. As a Social Worker, you will play a critical role in ensuring patients are discharged safely and identifying any barriers to care.
Key Responsibilities- Conduct thorough assessments of patients' social determinants of health, trauma certification, crisis intervention, abuse and neglect, and complexities within a Level 1 trauma center emergency department.
- Develop and maintain working relationships with community health, welfare, and social agencies to assist patients with their needs.
- Collaborate with the multi-disciplinary team to support patients' treatment and provide creative solutions to address their challenges.
- Support 24/7 on-site availability at the Level 1 trauma center.
- Master's degree in Social Work
- No experience required
