Prokatchers is seeking a skilled Travel Nurse RN Case Manager to join our team in Geneva, New York.
Job DescriptionAs a Travel Nurse RN Case Manager, you will be responsible for reviewing and evaluating assessments and UAS information for members in the MLTC and MAP lines of business. You will develop a working relationship with PCPs to discuss the care of members and identify any concerns that have not been identified by the clinical team. You will also identify risk factors and assign risk categories to members, develop formal care plans, and monitor the condition of members at least monthly.
Requirements- Bachelor's Degree required; BSN preferred
- 5 years healthcare-related experience, prior Care Management or Case Management experience in a hospital setting
- RN Licensed as a Registered Nurse in the State of New York required
