The Role
We're seeking a Registered Nurse (RN) Case Manager to join our cross-functional care team. As a key member of this team, you'll be responsible for providing clinical expertise and integrated health approaches to guide patients through complex healthcare situations. Your education and experience will enable you to apply nursing processes to actively manage patients and achieve coordinated health services.

Your Responsibilities
• Apply nursing processes to actively manage patients and achieve coordinated health services
• Provide education and guidance to patients and providers to navigate healthcare complexities
• Maintain high-quality care by adhering to therapeutic standards and measuring health outcomes against patient goals and standards
• Document all activities specific to patients, caregivers, providers, facilities, and clients in appropriate databases
• Collaborate with healthcare teams through effective communication, responsiveness, and problem-solving

Requirements
• Bachelor of Science in Nursing (BSN) preferred
• 2 years of clinical experience with direct patient care required
• Comfortable communicating with patients and providers via phone throughout the day
• Strong administrative/technical skills, including proficiency in Microsoft Office and telephones
• Trustworthy and accountable behavior, capable of maintaining confidential information daily
