We are seeking a highly skilled Registered Nurse to join our multidisciplinary care team as a Primary Care Nurse Coordinator at Medical Search International. This role requires a nurse with sound clinical judgment and critical thinking skills to provide high-quality primary care to patients with complex medical and psychiatric conditions.

Job Summary:

The Primary Care Nurse Coordinator plays a crucial role in supporting the primary care team in acute and chronic care management, conducting nursing visits, and care coordination. This position involves utilizing the nursing process for assessment, planning, implementation, and evaluation to provide safe and therapeutic care.

Responsibilities:
  • Supporting primary care providers during patient visits
  • Chart preparation, including medical records and medication lists
  • Attending clinic huddles and debriefs
  • New patient intakes
  • Conducting nursing visits and procedures such as immunizations, EKGs, PAP smears, I&Ds, joint injections, wound care, ear lavages, and point-of-care testing
  • Medication administration, including oral, injections, and nebulizer
  • Triage of in-person walk-ins and televisits
  • Nursing assessments and follow-up on hospital admission/ED visits and specialist visits
  • Follow-up on lab results, health screenings, and diagnostic imaging
  • Coordinating care with the multidisciplinary care team, external care providers, and other patient support
Requirements:
  • Active Registered Nurse licensure in the State of California
  • Active CPR/BLS certification
  • PREFERRED QUALIFICATIONS:
    Two years of RN clinical experience
    Experience in ambulatory care
    Experience working with serious mental illness populations
    Bachelor of Science in Nursing
    Knowledge of EPIC EHR, Clinician's Gateway EHR, and Microsoft office
Estimated Salary:

$85,000 - $105,000 per year, depending on experience and qualifications.
