Job Title: Case Planner

Rising Ground is a leading organization in the field of child welfare, providing innovative solutions to support vulnerable children and families. We are seeking a skilled Case Planner to join our team.

Job Summary:

The Case Planner will be responsible for providing constructive services to children in foster care, connecting parents to services that address family challenges, and assisting them with reuniting with their child. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with diverse stakeholders.

Key Responsibilities:
  • Provide direct case planning services to access child safety, child wellbeing, and facilitate permanency.
  • Maintain mandated contacts with caretakers, children, parents, siblings, clinical and medical staff, schools, and other resources.
  • Develop service plans and connect children and/or parents to appropriate services that address their individual needs.
  • Document progress notes and complete timely and accurate reports, including FASP, PH reports, and intra-agency reporting.
  • Make home visits and collateral contacts to meet ACS and OCFS requirements.
  • Attend Family Court hearings.
  • Facilitate and actively participate in six-month Service Plan Reviews and Family Team Conferences.
  • Establish a culturally appropriate network with other systems within the community.
  • Perform other related duties as assigned by supervisor.
Qualifications:
  • Bachelor's Degree and knowledge of the child welfare system.
  • 1 year of experience (preferred).
  • Good verbal and written communication skills are essential.
  • Computer proficiency and knowledge of Connections.
  • Bilingual (English/Spanish) a plus.
  • 3 Hybrid Days in the office & 2 Remote Days.
