POSITION SUMMARY
The Charge Nurse is accountable for overseeing the delivery of nursing care to a designated group of residents within a nursing unit. This role involves assessing resident requirements, formulating personalized care plans, administering nursing services, and evaluating the effectiveness of care provided. Additionally, the Charge Nurse supervises Care Specialists and other staff members, and may oversee fellow LPNs/LVNs.
KEY RESPONSIBILITIES
- Ensure the provision of high-quality nursing care that is focused on achieving positive outcomes through the application of the nursing process. Identify resident needs and contribute to discharge planning and follow-up throughout their stay.
- Collaborate with residents, their families, and the interdisciplinary care team to develop individualized care plans.
- Effectively implement care plans while utilizing available resources to support residents, families, or responsible parties in counseling and discharge planning. Work closely with physicians and other healthcare professionals to share relevant information regarding changes in care plans.
- Deliver and uphold optimal resident care and comfort by demonstrating proficiency in current nursing practices.
REQUIREMENTS
- Must possess and maintain a valid license to practice as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of practice.
- Recent clinical experience, education, or specialized skills in geriatrics are preferred.
- Ability to maintain regular attendance is essential.
SKILLS AND ATTRIBUTES
- Understanding of infection control principles and practices.
- Ability to maintain confidentiality regarding patient and organizational proprietary information.
- Capability to interact professionally and positively, fostering cooperation with residents, families, and colleagues at all levels.
