Responsibilities
  • Conduct thorough assessments of patients' physical and functional abilities to develop personalized treatment plans.
  • Implement and modify treatment plans as needed to ensure optimal patient progress and satisfaction.
  • Collaborate with patients, families, and healthcare professionals to ensure seamless care coordination and communication.
  • Document patient progress and treatment plans accurately and thoroughly, adhering to regulatory requirements and company standards.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • 1 year of experience in skilled home health, with a focus on occupational therapy.
  • BCLS certification required.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Strong problem-solving and critical thinking skills, with a patient-centered approach to care.
  • Proficiency in electronic medical records and other relevant software systems.
