About the Role

We are seeking a highly skilled and experienced Registered Nurse (RN) to join our team as a Clinical Supervisor in our Home Health division. As a Clinical Supervisor, you will play a critical role in ensuring the delivery of high-quality patient care and services to our clients.

Key Responsibilities
  • Perform on-site supervisory and teaching visits to evaluate caregivers and provide clinical orientation to caregivers on assigned home care cases.
  • Participate in the training of new nurses and in-services for field staff, ensuring they have the necessary skills and knowledge to provide exceptional patient care.
  • Conduct initial nursing assessments and counsel patients/caregivers on meeting nursing and related needs.
  • Assign and coordinate services provided, ensuring seamless care delivery.
  • Assist physicians in developing a plan of treatment/care and make necessary revisions based on patient condition/needs.
  • Prepare clinical progress notes, ensuring any changes in the patient's condition are reported.
  • Monitor clinical records for complete and accurate documentation.
Requirements
  • Registered Nurse (RN) licensure in the state of New York.
  • CPR certification.
  • At least one year of experience in home care, with a focus on skilled care settings.
  • Valid New York State driver's license, insurance, registration, and reliable vehicle.
