Job Title: MDS Coordinator RN

At NHC HealthCare Lexington, we are seeking a highly skilled and compassionate Registered Nurse (RN) to fill the role of MDS Coordinator. This is a full-time position that requires strong leadership and communication skills, as well as the ability to work effectively with interdisciplinary teams.

Key Responsibilities:
  • Conduct and document accurate assessments of patients to ensure timely and accurate completion of the MDS.
  • Interview patients and families to gather information and ensure an accurate picture of the patient's psychosocial/medical status.
  • Coordinate with other disciplines to ensure accurate and timely completion of the MDS.
  • Organize and prioritize tasks to meet deadlines and ensure efficient workflow.
  • Monitor the overall medical record to validate support of MDS coding.
  • Collaborate with the interdisciplinary team to ensure timely completion of Resident assessment Protocols and Patient Care Plans.
  • Communicate effectively with the bookkeeping department to ensure accurate and timely completion of tasks.
Requirements:
  • South Carolina RN (Registered Nurse) license.
  • Caring and compassionate attitude, with excellent communication skills and a positive attitude.
  • Experience as an RN is preferred, but LPNs with MDS experience will be considered.
