Job Summary:
Advocate Aurora Health is seeking a highly skilled Registered Nurse (RN) to join our Home Health team as a Case Manager. As a key member of our team, you will be responsible for providing high-quality patient care and coordinating services to ensure the best possible outcomes for our patients.
Responsibilities:
- Nurse/Patient Relationship: Develop and maintain a therapeutic relationship with patients and their families throughout the healthcare continuum.
- Assess and Plan: Assess patient and family readiness and involve them in planning and implementing care. Seek consultation as needed.
- Implement and Evaluate: Implement individualized care plans and evaluate patient progress. Make sound clinical decisions and prioritize patient care.
- Collaboration and Communication: Collaborate with healthcare team members to ensure seamless care coordination. Communicate effectively with patients, families, and healthcare providers.
- Leadership and Professional Development: Demonstrate leadership behaviors and participate in unit meetings and activities that improve patient care or administrative systems.
Requirements:
- Graduate from an accredited school of nursing
- Current Illinois RN license
- One year of medical-surgical experience
- Proficiency in patient care equipment and technology
- Excellent communication and interpersonal skills
- Ability to work weekends, holidays, and different shifts
- Ability to lift up to 100 pounds occasionally and/or in excess of 50 pounds frequently
