Job Overview

PRN Home Health & Therapy is seeking a skilled Physical Therapist Assistant to join our team in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Deliver Physical Therapy treatments to patients in accordance with treatment plans developed by the Physical Therapist.
  • Collaborate with the Interdisciplinary Team to identify and assist with obtaining patient-specific Durable Medical Equipment (DME), including mobility aids, wheelchairs, cushions, orthotics, and adaptive equipment.
  • Maintain accurate and detailed records of treatments and document progress notes on patients as directed by the Physical Therapist.
  • Work collaboratively with the Physical Therapist and clinical team to develop and educate patients and/or caregivers on safe discharge plans.
Requirements
  • Graduate of an accredited Physical Therapist Assistant program.
  • Current license/certification in Wisconsin.
  • Current American Heart Association CPR Certification.
