Job Summary

We are seeking a highly skilled Pharmacist to join our team in the Outpatient Pharmacy department. As a key member of our healthcare team, you will be responsible for providing medication management services to patients, working collaboratively with other healthcare professionals to deliver safe, effective, and cost-efficient medication therapy.

Responsibilities
  • Assess medication therapy regimens and develop personalized plans to optimize patient outcomes
  • Provide medication therapy management, order verification, and distribution services
  • Monitor medication therapy plans and provide education to patients and caregivers
  • Participate in quality improvement initiatives and contribute to research activities
Qualifications
  • Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program or Bachelor of Science (B.S.) with a minimum of 2 years relevant experience
  • Current pharmacist license by the Board of Pharmacy in the state(s) of the job's practice site, or eligible to obtain within a time frame compliant with the site's applicable employee licensure policy
