Job Description

The Case Manager plays a vital role in our human service team, working closely with residents to develop and execute personalized service and care plans. This position requires excellent communication and interpersonal skills, as well as the ability to prioritize tasks and make sound judgment calls.

Key Responsibilities

  • Coordinate with Social Workers to design and implement customized service and care plans for residents
  • Collaborate with external service providers to link residents with essential resources and support
  • Conduct regular unit visits to ensure healthy hygiene and housekeeping standards
  • Document interactions and communications with residents in CARES and AWARDS

Required Skills and Qualifications

  • High School Diploma or GED required; Bachelor's degree in social work or psychology preferred
  • Two years of experience in case management, counseling, or direct advocacy for clients in social service settings
  • Excellent verbal and written communication skills; proficiency in English required, multi-lingual and Spanish proficiency a plus
  • Basic competency in MS Word, Excel, Outlook, and the internet
