Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Leoforce. As a Licensed Practical Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Job Summary:

The Licensed Practical Nurse will be responsible for providing direct patient care, conducting clinical assessments, and documenting patient information. You will work closely with our interdisciplinary team to develop and implement patient care plans, ensuring the best possible outcomes for our patients.

Responsibilities:
  • Provide direct patient care, including administering medications, performing vital signs, and conducting clinical assessments.
  • Develop and implement patient care plans, in collaboration with the healthcare team.
  • Document patient information, including medical history, treatment plans, and progress notes.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
