Job Summary

The Lead Wellness Nurse is a key member of our team at Phoenix Senior Living, responsible for monitoring the health and wellbeing of our residents and providing leadership support and assistance with resident care and services.

This role requires a certified Licensed Practical Nurse or Registered Nurse with a minimum of one year of experience working with elders or disabled individuals. The ideal candidate will have demonstrated leadership capabilities and knowledge of assisted living industry and dementia care or skilled nursing.

Key Responsibilities:

  • Monitor resident health and wellbeing, identifying ongoing needs and services through the assessment and Personalized Service Plan (PSP) process.
  • Ensure proper follow-through and documentation for residents with a change in clinical care needs.
  • Support the Wellness Director with completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident's monthly status.
  • Inform the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out.
  • Assist with obtaining weights and vital signs monthly for each resident prior to completion of monthly wellness visit.
  • Provide oversight to staff and/or direction to other staff during crisis situations in the absence of the Wellness Director/Resident Care Director.
  • Review, read, and note the Daily Log to document and learn about pertinent information and any resident's changes. Ensure that day-to-day communication takes place between staff and between shifts.
  • Audit Incontinent/medical supplies and generate a list for the Wellness Director/Resident Care Director of items needed.
  • Ensure that the care stream tasks are signed off during each shift.
  • Assist with resident care as needed.
  • Be on call once a month.

Requirements:

  • Must be certified as a Licensed Practical Nurse or Registered Nurse.
  • A minimum of one year of experience working with elders or disabled individuals, preferred.
  • Must have demonstrated leadership capabilities.
  • Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred.
  • Ability to work weekends, evenings, and flexible hours, available to our customers at peak service delivery days and times.
