Job Summary

We are seeking skilled nursing and therapy professionals to join our team at Medacare Transportation Inc. as Home Health Care Providers. As a Home Health Care Provider, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Deliver Patient-Centered Care: Provide direct patient care, implement treatment plans, and work collaboratively with the healthcare team to achieve positive patient outcomes.
  • Assess and Document Patient Needs: Accurately and timely document patient information, assess patient needs, and provide education and support to patients and their families.
  • Collaborate with Healthcare Team: Participate in the coordination of home health services, report identified needs to the registered nurse and/or Clinical Supervisor, and assist in the development of care plans.
  • Provide Health Education: Counsel patients and families on meeting nursing and therapy needs, provide health care instructions, and identify discharge planning needs.
Requirements
  • Education and Experience: RNs, LPNs, and therapists with experience in home health care.
  • Skills and Qualifications: Strong communication and interpersonal skills, ability to work independently, and knowledge of OASIS and CMS requirements.
