About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager in Hospice Care. As a key member of our care team, you will be responsible for providing patient-centered care and collaborating with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Perform patient assessments and develop comprehensive care plans in collaboration with the care team.
  • Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
  • Develops, prepares, and maintains accurate and timely patient care progress records.
  • Teaches patients, families, and caregivers about patient care techniques and provides education on pain and symptom management.
  • Regularly communicates with clinical managers and care team members to ensure seamless care coordination.
  • Performs on-call responsibilities and provides on-call services to patients and families as assigned.
Qualifications
  • One year of nursing experience.
  • Current RN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
